Hubbard-Max-Quality

Maria Ramirez Hubbard, MD, FAAN has over 22 years of experience in Behavioral Neurology and Brain Injury Medicine. She completed her Neurology residency and Behavioral Neurology fellowship at Boston University, one of the leading TBI research institutions in the world where chronic traumatic encephalopathy (CTE) was discovered.

Dr. Hubbard is fluent in Spanish as well as conversational in French, allowing her to communicate effectively with many of our bilingual patients across the state.

Dr. Hubbard is one of 8 neurologists in the state of Florida who are board-certified in brain injury medicine by the American Board of Neurology and Psychiatry, and she also holds a Diplomate in Neurorehabilitation.

In addition to evaluating and treating patients, she also utilizes advanced diagnostic testing like VNG, balance testing, oculomotor tracking, and other technologies that can aid in the diagnosis and treatment of TBI. These tests often are positive even when MRIs or other imaging studies don’t show substantial findings.

In her spare time she enjoys spending time with her family and trying new recipes. She is involved with several organizations dedicated to the guidance and mentoring of young physicians, a cause for which she is very passionate.

Jakubek-High-Quality

Dr. Adam Jakubek is a 2008 graduate of Lake Erie College of Osteopathic Medicine and a Diplomate of the American Board of Psychiatry and Neurology.  As a board-certified neurologist, he has a special interest in a range of neurological disorders ranging from peripheral nerve injuries to traumatic brain injury (TBI). 

Dr. Jakubek completed his neurology residency at the prestigious Walter Reed Army Medical Center, where he had extensive experience in the evaluation and treatment of TBI.

He is married with three children and enjoys running and cycling when he is not working.

Dr Asad

Dr. Asad serves as a consulting neurologist for the Jacksonville Jaguars, and has completed residencies at Harvard and Emory 
University. 
 
Prior to opening his private practice in 2016 Dr. Asad was chief of neurology at Baptist 
Medical Center in Jacksonville. With experience as a Captain in the U.S. Army Reserves, he knows what it takes to be an efficient leader and care-giver.
 
Dr. Asad is one of 10 neurologists in the state of Florida who are board certified in brain injury medicine by the American Board of Neurology and Psychiatry, and TBI patients make up a majority of his practice. 
 
He is also board certified in neurology and nuclear medicine, and maintains hospital privileges at Baptist Medical Center in Jacksonville.
 
In addition to evaluating and treating patients he also utilizes advanced diagnostic testing like VNG, EEG with ERPs, oculomotor tracking, and other technologies that can aid in the diagnosis and treatment of TBI. These tests often are positive even when MRIs or other imaging studies don’t show substantial findings.
 
Outside of the office, Dr. Asad enjoys spending time with his wife and two daughters. He also loves skiing, traveling, and the occasional outdoor adventure, ranging from climbing Mt. Kilimanjaro to swimming with sharks in the Atlantic Ocean.
 

Ebert 1

Dr. Lacey Ebert grew up in a small town in rural Virginia. In high school, she made Honorable Mention All-District Volleyball, was Drum Major of the marching band, and was one of the founding team members of the Pocahontas High School tennis team. Dr. Ebert began her undergraduate studies at Sweet Briar College, during which time she worked with horses and took up fencing. She completed her undergraduate degree in Pre-Professional Biology at Appalachian State University, where she was an officer in Kappa Kappa Psi, a co-ed fraternity, dedicated to service for the school's bands. 

Dr. Ebert attended National University of Health Sciences in Lombard, Illinois, graduating with her Doctorate of Chiropractic in 2013. She continued her pursuit of knowledge, attaining a Master of Science in Acupuncture in 2016. During this time, she interned at Cook County Hospital in Chicago, Illinois, working in the pain management clinic and providing acupuncture for the underserved. After graduating in 2016, Dr. Ebert began a clinic in Oak Park, Illinois, where she offered community acupuncture and free acupuncture for military veterans suffering from PTSD. 

Dr. Ebert joined Florida Spine and Injury in 2018, moving from Chicago to escape the cold weather. She is currently pursuing a Diplomate of the American Board of Chiropractic Internists, and is looking forward to better serve patients and treat the whole person.

Dr. Tony Sancetta is a graduate of Nova Southeastern University College of Osteopathic Medicine and has over 15 years of experience treating patients with musculoskeletal injuries such as back pain, beck pain, headaches, sciatica, disc herniations, and whiplash.

After graduating from medical school, Dr. Sancetta participated in a post-graduate fellowship in Manual Medicine. He believes in treating the total person, not just symptoms, and has a genuine concern for the health of his patients.

 

Walker (2)

Dr. Jonathan Walker has been practicing in Florida since 2008 and is proud to focus on the treatment of auto accident injuries, peripheral neuropathy, disc injuries, and knee pain.

In 2009, he founded the first multidisciplinary clinic in Marion County, where chiropractors and medical physicians worked together under one roof.

Dr. Walker is a graduate of the University of Florida as well as Palmer College of Chiropractic Florida and has completed extensive post-graduate training in the field of whiplash, car accident injuries, ligament trauma, and traumatic brain injury.

He loves spending time with his wife Jennifer and his children Ellie, Corban, and Hudson.

Dr. Walker teaches Sunday school at his church, Trinity Baptist, and enjoys skiing, traveling, and training other doctors across the country.

Wood-High-Quality

Dr. Wood, D.C., grew up in Lake City and has practiced in Lake City and Live Oak since 2003. He
graduated with a Doctor of Chiropractic Degree from Life University in 2001.

He is a member of the Florida Chiropractic Association and North Florida Chiropractic Society.  The main focus of his treatments is auto accident injuries, disc injuries, and neck and back pain.

He enjoys keeping up with the latest research and advancements in the treatment of whiplash injuries.

Dr. Wood is married with two children and enjoys playing golf and fishing in his free time.

Kyle Chacon

Brittney Lewis-High-Quality

Dr. Brittany Lewis lives in St. Pete and is a Traumatic Brain Injury Tester for the Tampa Bay area offices. She enjoys testing and working with patients to help paint a picture toward their rehabilitation goals. 

Dr. Lewis went to school in Connecticut at the University of Bridgeport School of Chiropractic. She graduated magna cum laude and received the Clinical Excellence Award, honored in recognition of outstanding patient care. Dr. Lewis also teaches yoga and enjoys the close relationship between her two careers. Feel free to show off your down dog when you meet her.

Craig Siefkes-High-Quality

Originally from central Kansas, Dr. Craig Siefkes attended Kansas State University, graduating with a bachelor’s degree in Psychology. He continued on to Palmer College of Chiropractic in Davenport, IA, where he received his Doctor of Chiropractic in 2009.

Dr. Siefkes has practiced in central Pennsylvania, Austin, TX, and central Florida. He is certified in Activator Methods and enjoys learning more about Thompson technique.

In his free time, Dr. Siefkes enjoys going to Disney World with his wife and daughter, as well as watching hockey, college football, and theater productions. He is also a huge fan of Star Wars, Star Trek, and Doctor Who.

 

Katelyn Self

Dr. Katelyn Self grew up in a small town just outside of Colorado Springs, CO. She has always had a passion for healthcare. Her early endeavors included lifeguarding, taking EMT-Basic and Certified Nursing Assistant classes, being a student athletic trainer, and a Chiropractic Assistant.

She completed her undergraduate studies at Colorado State University where she received her Bachelor of Science in Health and Exercise Science, with a concentration in Sports Medicine. Dr. Self followed her love of healthcare and continued her education at Palmer College of Chiropractic's Florida campus. She graduated Magna Cum Laude, earning her Doctor of Chiropractic in 2016.

Dr. Self lives in Port Orange and is a Traumatic Brain Injury Tester for the East Coast offices. She enjoys testing patients, compassionately listening to their stories, and helping them to have a better understanding of their brain injury to move forward towards their rehabilitation goals.

In her free time, Katelyn loves to spend time with family and friends, run, hike, and just enjoy the outdoors; from the beaches of Florida to the mountains of Colorado.

Shanklin

Matt Shanklin, DPT, LMT, earned his Doctorate in Physical Therapy from Nova Southeastern and has
practiced in Tampa Bay his entire career.

Previously working in sports massage for distance runners and triathletes, Matt specializes in manual therapy for all musculoskeletal injuries. He has worked closely with leading Chiropractors in Tampa Bay to best understand the needs of injury patients as well as those with specific nerve pathologies including Neuropathy.

Joining Ethos in St. Petersburg and Palm Harbor, he employs an evidence and experience-based
approach to patients suffering from Neuropathy, Knee Pain, and injuries sustained from accidents and trauma.

Shanklin lives in St. Petersburg with his wife and daughter and can be found participating in local triathlons and spending time with his family.

Winona Werts, APRN-C, FNP, grew up in Rockledge, FL. She attended the University of South Florida where she received her Bachelor of Science in Nursing Degree.

She served as an RN in Columbus, OH for 5 years, caring for pediatric patients at the Nationwide Children’s Hospital. She continued her
education at the University of Cincinnati where she completed her Master of Science in Nursing
Degree in the Family Nurse Practitioner Program.

As a Nurse Practitioner for Ethos Health Group, Winona brings with her a background in Internal
Medicine and Primary Care.

 

Wolfersberger-High-Quality

Graduating in one of the inaugural classes at the Florida campus of Palmer Chiropractic College Dr. Joseph Wolfersberger, D.C. has been in practice for over a dozen years. Chiropractic has been part of his life since childhood in a small beach town on the Jersey Shore. In his senior year in high school a rough tackling drill at football practice caused a severe “whiplash injury” that required a longer course of treatment. It was a wonderful experience recovering from the injury, see others get better. This great experience set him on the path to becoming a Doctor of Chiropractic.

After high school, he decided to relocate to Jacksonville to attend the University of North Florida. After earning a bachelor's degree in Spanish from the University of North Florida, he worked in the banking and communication industries until a layoff from a job led him to reassess his career choice. He went back on his original path to becoming a chiropractor.

After becoming a Chiropractor, he was hired on with one of the most respected, experienced, and longest continuously operating clinics in the state of Florida. He then covered at several different chiropractic offices around North and Central Florida until finding his perfect fit with Florida Spine and Injury in Lake City.

Dr. Wolfersberger has 3 daughters, two of whom are adults and one pre-teen still at home. In his spare time he likes to do yoga, outdoor activities such as camping, hiking, riding bikes and kayaking/canoeing. Being a “beach kid” with shoes full of sand, he loves spending time at the beach or at one the many springs around northern and central Florida whenever possible.

Jennifer has earned her Masters in Science of Nursing through South University. She is certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners (AANP). Though she is a recent FNP graduate, January of 2020, she has a strong background working over 11 years in Emergency/Critical care, medical and surgical nursing, and even rehabilitation. She enjoys living with her family in Edgewater, FL; where she has resided the majority of her life. She enjoys being outdoors in the Florida sun and entertaining her 3 children. She believes in developing a strong rapport and trusting relationship with her patients in order to provide them with the care they deserve. She ensures every individual gets specialized care tailored to meet their individual needs.

Dr. Crist Francisco was raised near Ft. Lauderdale, Florida. His mother, a massage therapist, taught him all the bones in the body at the age of 4. From a very young age, he competed in just about every sport possible throughout his childhood. Crist's father taught him the game of golf at 10, and he fell in love with the game. He played competitive golf throughout high school and college at Johnson & Wales University. He later went on to compete in professional events in the South Florida area and also taught golf for 6 years.

In 2007, Dr. Francisco finished his BSBA at Nova Southeastern University, and also earned his Florida real estate license (still active today). In 2010, he went on to earn a MA in Youth & Young Adult Ministry from Andrews Theological Seminary in Berrien Springs, Michigan. There he worked as a youth pastor and was director of the Pioneer Memorial Church summer camp in 2010. He currently holds a unit of CPE training which allows him to serve as a hospital chaplain in his spare time. He decided to pursue his dream in 2011 and began attending Palmer College of Chiropractic in Port Orange, FL. He graduated with his Doctor of Chiropractic in 2014. He has worked at various chiropractic clinics since graduating school in Deltona, FL, Edgewater, FL, West Palm Beach, FL and now in lovely NSB. He has taught classes as a part time faculty member at the Brilliant Nursing Academy in Orlando, FL. He has gained additional certifications and/or training in the following chiropractic techniques:

He has provided personal training for clients, and took up bodybuilding in his 20s. This additional knowledge of the human anatomy has greatly aided him in his work and development of new and innovative osseous and soft tissue therapies. He has also been the patient a few times in his life, most recently a few years ago when herniated discs and sciatic pain almost stopped him from being able to practice chiropractic. He has worked on himself and many others and been able to help heal the body through nutritional knowledge and therapies.

He enjoys working out, eating out, playing golf, surfing, the Florida Gators, and playing sports whenever possible. He's happy to be living and serving up yummy ice cream & Italian Ice (when not in the office) near the world's most famous beach in Daytona Beach Shores with his wife Christina and daughter.

Dr. Sarah Brooks has always had a passion for healthcare. Beginning her career in medicine in sunny Naples, Florida where she worked in a chiropractic office, she was inspired to earn her doctorate at Palmer College of Chiropractic in Daytona, Florida. During her time as a student, Dr. Brooks developed a particular appreciation for the complexity of the musculoskeletal system. Now, she brings to Ethos Health Group years of experience and a love of connecting with her patients that shows in her caring and positive attitude. In her free time, Dr. Brooks enjoys spending time with her family and friends, and visiting the many theme parks central Florida has to offer. Her son Bruce will be the first to tell you: "My mom is awesome!"
