We are proud to have Dr. Asad on our brain injury team as a part of our TeleTBI program. He serves as a consulting neurologist for the Jacksonville Jaguars and has completed residencies at Harvard and Emory University. Prior to opening his private practice in 2016, Dr. Asad was chief of neurology at Baptist Medical Center in Jacksonville. With experience as a Captain in the U.S. Army Reserves, he knows what it takes to be an efficient leader and care-giver.

Dr. Asad is one of 8 neurologists in the state of Florida who are board-certified in brain injury medicine by the American Board of Neurology and Psychiatry, and TBI patients make up a majority of his practice. He is also board-certified in neurology and nuclear medicine and maintains hospital privileges at Baptist Medical Center in Jacksonville.

In addition to evaluating and treating patients, he also utilizes advanced diagnostic testing like VNG, EEG, oculomotor tracking, and other technologies that can aid in the diagnosis and treatment of TBI. These tests often are positive even when MRIs or other imaging studies don’t show substantial findings.

Outside of the office, Dr. Asad enjoys spending time with his wife and two daughters. He also loves skiing, traveling, and the occasional outdoor adventure, ranging from climbing Mt. Kilimanjaro to swimming with sharks in the Atlantic Ocean.

Azeem

 

Dr. Azeem is an associate assistant professor in the pain/neurology department of the University of South Florida, and actively continues to conduct research and author peer-reviewed publications. In addition to treating patients he is the President of the Florida Society of Pain & Neuroscience, Chair of Strategic Development for State Society Engagement and Advocacy, and a member of the American Society of Pain & Neuroscience and Board of Directors.

Anthony Sancetta DO

 

Dr. Tony Sancetta is a graduate of Nova Southeastern University College of Osteopathic Medicine and has over 15 years of experience treating patients with musculoskeletal injuries such as back pain, beck pain, headaches, sciatica, disc herniations, and whiplash.

After graduating from medical school, Dr. Sancetta participated in a post-graduate fellowship in Manual Medicine. He believes in treating the total person, not just symptoms, and has a genuine concern for the health of his patients.

Dr. Jonathan Walker has been practicing in Florida since 2008 and is proud to focus on the treatment of auto accident injuries, peripheral neuropathy, disc injuries, and knee pain.

In 2009, he founded the first multidisciplinary clinic in Marion County, where chiropractors and medical physicians worked together under one roof.

Dr. Walker is a graduate of the University of Florida as well as Palmer College of Chiropractic Florida and has completed extensive post-graduate training in the field of whiplash, car accident injuries, ligament trauma, and traumatic brain injury.

He loves spending time with his wife Jennifer and his children Ellie, Corban, and Hudson.

Dr. Walker teaches Sunday school at his church, Trinity Baptist, and enjoys skiing, traveling, and training other doctors across the country.

Fontaine

 

Dr. Richard Fontaine III is a board-certified Chiropractic Physician in the state of Florida. He is a graduate of Palmer College of Chiropractic's Florida campus.

Dr. Fontaine is a native Rhode Islander that has decided to remain with his family here in the sunshine state.

He enjoys hiking, fishing, and exploring the many natural springs of Florida with his family.
Dr. Fontaine has been a member of our Ocala Spine & Injury family (formerly Ocala Integrative Medicine) since 2011.

He is a proud father and enjoys keeping his family, and our patients well adjusted with regular chiropractic care. Even if you are enjoying your first days of life, he has studied several techniques to provide safe and gentle care for people of all ages.

He practices Palmer package techniques, including diversified, instrument, Gonstead, and Thompson drop. Additional studies and techniques include further studies into the Thompson technique and the certified chiropractic extremity practitioner program for better evaluation and management of the extremities.

Irina Baum

 

Irina Baum, APRN, was raised in Kazakhstan and came to the United States to conquer "the world of
opportunities and prosperity.”

She joined the Army National Guard and served 6 years as a combat medic where she learned to care for casualties in the field. She graduated from the University of MD School of Nursing with a Bachelor of Science in Nursing Degree and worked for Johns Hopkins Hospital as a medical/surgical and critical care nurse. She acquired her Nurse Practitioner training at Walden University before joining Ethos Health Group.

Irina enjoys gardening, swimming, and cooking. She is fluent in both English and Russian and loves
working with adults and senior citizens.

Dr. Centra graduated from Chatham University’s Doctor of Physical Therapy program and is a licensed Physical Therapist in the state of Maryland.

He has a Bachelor of Science Degree in Rehabilitation Science and a Certificate of Pathokinesiology in Rehabilitation from the University of Pittsburgh.

He is also certified as a Personal Trainer (CPT), Fitness Nutrition Specialist (ACE-FNS), and Strength and Conditioning Specialist (CSCS).

Along with his work at Ethos Health Group, Dr. Centra is an independent contractor for the Academy of Neurologic Physical Therapy where he programs online coursework for medical professionals.

Outside of the office, Jake is passionate about the environment, music, real food, and fitness.

Jasmin Eusebio, PT, graduated in 2001 with a Bachelor of Physical Therapy Degree in the Philippines.

She has treated patients as a Physical Therapist both inside and outside the Philippines since 2001.
She attended the Philippine Rehabilitation Institute College of Sciences to obtain her Master of Physical Therapy Degree in 2015.

Eusebio passed her National Physical Therapy examination in California with her original license
issued in New York, obtaining her Maryland license in 2018.

As a clinician, she places emphasis on post-operative protocols and geriatric rehabilitation programs, providing individualized treatment for her patients.

Heidi Hurter, APRN, attended South University for her Bachelor and Master of Science Degrees in
Nursing. She later attained her Nurse Practitioner Degree with a desire to have a more positive impact
on patient's lives.  She began her nursing career as a hospice nurse and then became a mother/baby
nurse.

She is trained to inject the entire body with ozone, stem cells, and related medicines and has brought immense benefits for her patients.

Originally from Sarasota, FL, Heidi is interested in regenerative, holistic, and functional medicine and
believes in treating all people with kindness and without judgment.

Meagan Johnson, DNP, APRN, FNP-BC, is a Florida native, earning her Bachelor of Health
Science Degree from the University of Florida. She attended the University of Florida’s accelerated Nursing Program, earning a Bachelor of Science Degree in Nursing.

As a Registered Nurse, Meagan worked in the Trauma Medical-Surgical Unit at UF Health Shands Hospital for over five years. She graduated from the University of Florida with a Doctor of Nursing Practice Degree.

A board-certified Family Nurse Practitioner, Meagan is passionate about patient-centered care with a holistic approach for patients of all ages.

Meagan enjoys spending time with her family and fiancé, a fellow nurse practitioner. She loves
traveling and cheering for the Florida Gators.

Diana Linn, PT, DPT, earned her Bachelor of Science Degree in Biological Sciences in 2008 from the University of Maryland, Baltimore County. She continued her education at the University of Maryland, Baltimore, earning her Doctorate in Physical Therapy in 2011.

Diana has spent most of her Physical Therapy career treating and working with seniors. Diana is
passionate about using her background of movement science and wellness to create individualized plans for her patients so they can return to the activities they love.

In her free time, Diana enjoys yoga, being outside, and spending time with friends and family.

Daisuke Matsuo MSN, FNP-C
 
Daisuke Matsuo, MSN FNP-C, earned his Associate Degree in Nursing from the Community College of
Baltimore County in 2010. He started working at the Emergency Room at Bon Secors Hospital in
Baltimore, and later at the Emergency Room at Northwest Hospital in Randallstown, MD. He later
earned his Bachelor of Science Degree in Nursing from Coppin State University and his Master of
Science in Nursing with a focus in Family Nurse Practitioner from the University of Cincinnati.  He
officially became a Licensed Nurse Practitioner in 2018. He has since worked in Urgent Care,
Pediatrics, and Community Nursing.
He joined the talented team of Providers at Ethos Health Group at Owings Mills in 2018, caring for
patients with Osteoarthritis and Neuropathy.

Dr. Jessica Perhealth was born in Ocala, FL and graduated from Belleview High School. She was an avid member of the softball team which led to her excelling her athletic career at Central Florida State College, followed by Bluefield State College, where she graduated with a Bachelor of Science degree in Interdisciplinary Sciences and a minor in Biology.

While at Bluefield State, Dr. Perhealth was a dedicated softball and volleyball player, president of the Pre-med Club and the Newman Club, a member of the Delta Chi Omega sorority, spent time volunteering at a local chiropractor's office as well as at the area hospital.

Dr. Perhealth decided to follow her love of healthcare and earned her Doctor of Chiropractic degree; graduating from Palmer College of Chiropractic in Port Orange, FL in March 2012.

Dr. Perhealth uses Palmer package adjusting technique: Gonstead, Diversified, Thompson, as well as Flexion/Distraction and activator.

Jessica and her husband, Mike Perhealth, reside in Summerfield with their four children, Myer, Paisley, Wyatt and Westin. They are both local residents of Marion County and are proud to serve their community.

Caitlyn Royal, DPT, is a compassionate and motivated Doctor of Physical Therapy providing
rehabilitative services to Sarasota and Hillsborough counties since August 2019.

She earned her Bachelor of Science in Health Sciences with a minor in Therapeutic Recreation from Utica College in Central New York before moving to South Florida to pursue her Doctorate Degree in Physical Therapy.

She graduated from Florida Gulf Coast University in May 2019, eager to follow her dream of helping
others as a Physical Therapist.

Caitlyn joined Ethos Health Group to help those living in our community who seek to enrich their lives through improved mobility and confidence while building lasting therapeutic relationships.

Dr. Sabatose, D.C., D.I.C.B.N., grew up in a small, rural Pennsylvania town called Brockway.

In high school, he played soccer, basketball, and cross country. He completed his undergraduate studies at Allegheny College where he majored in Biochemistry and minored in Psychology.

Dr. Sabatose attended chiropractic school in Port Orange, FL at Palmer College of Chiropractic where he graduated as the Valedictorian of his class. Dr. Sabatose has undergone post-graduate specialization in the field of Clinical Neuroscience, and he has his board certification from the International Board of Chiropractic Neurology.

Dr. Sabatose enjoys going to the beach, eating, and spending time with family and friends.

Tamika Scott

 

Tamika Scott, MSN, AGNP-C, received her Master of Science in Nursing Degree from the University of
Cincinnati with a specialization in Adult-Gerontology primary care.

Board-certified by the American Academy of Nurse Practitioners, Tamika Scott is experienced in providing primary care to patients with acute and chronic medical conditions. She currently provides specialized treatment for patients with Peripheral Neuropathy and Knee Pain caused by Chronic Osteoarthritis.

As an inpatient nurse clinician, Tamika spent 9 years providing skilled care for critically ill patients suffering from respiratory, renal, and hepatic failure in the Medical ICU at Johns Hopkins Hospital in Baltimore, MD.

Married with two boys, Tamika enjoys reading, traveling, exercising and spending time with her
family.

Shanklin

 

Matt Shanklin, DPT, LMT, earned his Doctorate in Physical Therapy from Nova Southeastern and has
practiced in Tampa Bay his entire career.

Previously working in sports massage for distance runners and triathletes, Matt specializes in manual therapy for all musculoskeletal injuries. He has worked closely with leading Chiropractors in Tampa Bay to best understand the needs of injury patients as well as those with specific nerve pathologies including Neuropathy.

Joining Ethos in St. Petersburg and Palm Harbor, he employs an evidence and experience-based
approach to patients suffering from Neuropathy, Knee Pain, and injuries sustained from accidents and trauma.

Shanklin lives in St. Petersburg with his wife and daughter and can be found participating in local triathlons and spending time with his family.

Jessica Simone, APRN, is a board-certified Adult-Gerontology Nurse Practitioner. She earned a
Bachelor of Science in Nursing Degree from the University of Connecticut in 2006.

She enjoyed eight fulfilling years serving in the Veterans Health Administration in various specialties including medical, surgical, and oncology care as well as in quality management. A veteran of the armed forces, Jessica
obtained her Master of Science in Nursing Degree from South University and holds a national
certification with the American Academy of Nurse Practitioners (AANP) as an Adult-Gerontology
Nurse Practitioner.

In her role as an Adult-Gerontology Nurse Practitioner, she is compassionate and works closely with her patients to determine their best, individualized care plan.

Winona Werts, APRN-C, FNP, grew up in Rockledge, FL. She attended the University of South Florida where she received her Bachelor of Science in Nursing Degree.

She served as an RN in Columbus, OH for 5 years, caring for pediatric patients at the Nationwide Children’s Hospital. She continued her
education at the University of Cincinnati where she completed her Master of Science in Nursing
Degree in the Family Nurse Practitioner Program.

As a Nurse Practitioner for Ethos Health Group, Winona brings with her a background in Internal
Medicine and Primary Care.

Dr. Wood, D.C., grew up in Lake City and has practiced in Lake City and Live Oak since 2003. He
graduated with a Doctor of Chiropractic Degree from Life University in 2001.

He is a member of the Florida Chiropractic Association and North Florida Chiropractic Society.  The main focus of his treatments is auto accident injuries, disc injuries, and neck and back pain.

He enjoys keeping up with the latest research and advancements in the treatment of whiplash injuries.

Dr. Wood is married with two children and enjoys playing golf and fishing in his free time.
