Maria Ramirez Hubbard, MD, FAAN has over 22 years of experience in Behavioral Neurology and Brain Injury Medicine. She completed her Neurology residency and Behavioral Neurology fellowship at Boston University, one of the leading TBI research institutions in the world where chronic traumatic encephalopathy (CTE) was discovered.

Dr. Hubbard is fluent in Spanish as well as conversational in French, allowing her to communicate effectively with many of our bilingual patients across the state.

Dr. Hubbard is one of 8 neurologists in the state of Florida who are board-certified in brain injury medicine by the American Board of Neurology and Psychiatry, and she also holds a Diplomate in Neurorehabilitation.

In addition to evaluating and treating patients, she also utilizes advanced diagnostic testing like VNG, balance testing, oculomotor tracking, and other technologies that can aid in the diagnosis and treatment of TBI. These tests often are positive even when MRIs or other imaging studies don’t show substantial findings.

In her spare time she enjoys spending time with her family and trying new recipes. She is involved with several organizations dedicated to the guidance and mentoring of young physicians, a cause for which she is very passionate.


Dr. Adam Jakubek is a 2008 graduate of Lake Erie College of Osteopathic Medicine and a Diplomate of the American Board of Psychiatry and Neurology.  As a board-certified neurologist, he has a special interest in a range of neurological disorders ranging from peripheral nerve injuries to traumatic brain injury (TBI). 

Dr. Jakubek completed his neurology residency at the prestigious Walter Reed Army Medical Center, where he had extensive experience in the evaluation and treatment of TBI.

He is married with three children and enjoys running and cycling when he is not working.


Dr. Jollu is a diplomate in American Board of Pain Medicine and is also certified in Physical Medicine & Rehabilitation. He is a fellow at International Spine Intervention Society and Fellow Member of the American Academy of Physical Medicine & Rehabilitation. He completed his residency training at Montefiore Medical Center affiliated to Albert Einstein College of Medicine in New York City, NY.

He completed a fellowship in Interventional Spine Care and Sports Medicine Injuries at the Michigan Sports and Spine Center in 2008. He is trained in the most advanced and minimally invasive interventional pain procedures.

Dr. Jollu believes in a holistic approach to treating his patients, with an overall goal to eliminate pain and restore and improve the patient's quality of life.

DR Azeem

Nomen Azeem, MD, FAAPMR is an interventional pain specialist and sports medicine specialist.

Dr. Azeem completed his undergraduate education at James Madison University in
Harrisonburg, Virginia, and graduated from medical school, earning his Doctor of Medicine from Ross University School of Medicine. After that, he completed an internship at Medstar Harbor Hospital Center in Baltimore and a residency at Medstar Georgetown University Hospital Physical Medicine and Rehabilitation in Washington, DC, where he was the chief resident. During this time, he was able to treat patients with various musculoskeletal injuries, complex traumatic brain injuries, and spinal cord injuries.

After filling a faculty position at Medstar Georgetown University as a musculoskeletal medicine specialist, he went on to complete an ACGME interventional pain fellowship at Temple University Hospital in Philadelphia, under the guidance of internationally renowned pain physician Dr. Frank J.E. Falco.

Dr. Azeem served as a board member for the Greater New Orleans Sports Foundation and held the position of director of neuromodulation in a multidisciplinary pain practice in New Orleans before moving to Florida where he developed the first successful non-opioid based interventional pain clinic in the Sarasota, Florida area.

His background helped shape Dr. Azeem’s approach to treating pain, and he includes both patients and their families in developing treatment plans so that everyone involved understands the origin of the patient’s pain as well as their available treatment options.

Dr. Azeem actively continues to conduct research and author peer-reviewed publications in addition to treating patients. He continues to stay involved with multiple pain societies to help advance the field of interventional pain medicine. He is the current president of the Florida Society for Pain and Neuroscience, board member of the American Society for Pain and Neuroscience, and board member of the Florida Prescription Drug Monitoring Program Foundation.


Dr. Tony Sancetta is a graduate of Nova Southeastern University College of Osteopathic Medicine and has over 15 years of experience treating patients with musculoskeletal injuries such as back pain, beck pain, headaches, sciatica, disc herniations, and whiplash.

After graduating from medical school, Dr. Sancetta participated in a post-graduate fellowship in Manual Medicine. He believes in treating the total person, not just symptoms, and has a genuine concern for the health of his patients.


Walker (2)

Dr. Jonathan Walker has been practicing in Florida since 2008 and is proud to focus on the treatment of auto accident injuries, peripheral neuropathy, disc injuries, and knee pain.

In 2009, he founded the first multidisciplinary clinic in Marion County, where chiropractors and medical physicians worked together under one roof.

Dr. Walker is a graduate of the University of Florida as well as Palmer College of Chiropractic Florida and has completed extensive post-graduate training in the field of whiplash, car accident injuries, ligament trauma, and traumatic brain injury.

He loves spending time with his wife Jennifer and his children Ellie, Corban, and Hudson.

Dr. Walker teaches Sunday school at his church, Trinity Baptist, and enjoys skiing, traveling, and training other doctors across the country.


Dr. Richard Fontaine, III, Director of Practice Analytics, is a board-certified Chiropractic Physician in the state of Florida. He is a graduate of Palmer College of Chiropractic's Florida campus.
Dr. Fontaine is a native Rhode Islander who resides with his wife and three children here in the sunshine state. He enjoys hiking, fishing, and exploring the many natural springs of Florida with his family.

Dr. Fontaine has been a member of our Ocala Spine and Injury family since 2011. He enjoys keeping his family, and our patients, well-adjusted with regular chiropractic care. Even if your child is enjoying their first days of life, he has studied several techniques to provide safe and gentle care for people of all ages.

He practices Palmer package techniques, including diversified, instrument, Gonstead, and Thompson drop. Additional studies and techniques include further studies into the Thompson technique and the certified chiropractic extremity practitioner program for better evaluation and management of the extremities.

Ela Davis-High-Quality

Ela J. Davis completed her Masters degree as a Family Nurse Practitioner at The Ohio State University. She has experience in integrative functional medicine, primary care, and cardiology.

Her goal is to provide her patients with individualized care that is thoughtfully planned to meet their specific needs. She takes the time to listen to and get to know each of her patients, while also motivating them to improve their health.

Ela enjoys swimming and traveling with her family. She volunteers with her daughter at the Humane Society.

Ebert 1

Dr. Lacey Ebert grew up in a small town in rural Virginia. In high school, she made Honorable Mention All-District Volleyball, was Drum Major of the marching band, and was one of the founding team members of the Pocahontas High School tennis team. Dr. Ebert began her undergraduate studies at Sweet Briar College, during which time she worked with horses and took up fencing. She completed her undergraduate degree in Pre-Professional Biology at Appalachian State University, where she was an officer in Kappa Kappa Psi, a co-ed fraternity, dedicated to service for the school's bands. 

Dr. Ebert attended National University of Health Sciences in Lombard, Illinois, graduating with her Doctorate of Chiropractic in 2013. She continued her pursuit of knowledge, attaining a Master of Science in Acupuncture in 2016. During this time, she interned at Cook County Hospital in Chicago, Illinois, working in the pain management clinic and providing acupuncture for the underserved. After graduating in 2016, Dr. Ebert began a clinic in Oak Park, Illinois, where she offered community acupuncture and free acupuncture for military veterans suffering from PTSD. 

Dr. Ebert joined Florida Spine and Injury in 2018, moving from Chicago to escape the cold weather. She is currently pursuing a Diplomate of the American Board of Chiropractic Internists, and is looking forward to better serve patients and treat the whole person.


Jasmin Eusebio, PT, graduated in 2001 with a Bachelor of Physical Therapy Degree in the Philippines.

She has treated patients as a Physical Therapist both inside and outside the Philippines since 2001.
She attended the Philippine Rehabilitation Institute College of Sciences to obtain her Master of Physical Therapy Degree in 2015.

Eusebio passed her National Physical Therapy examination in California with her original license
issued in New York, obtaining her Maryland license in 2018.

As a clinician, she places emphasis on post-operative protocols and geriatric rehabilitation programs, providing individualized treatment for her patients.

Michael Gardner-High-Quality

Michael Gardner PT, DPT is a native of Louisville, Kentucky.  After earning his Bachelor of Science Degree in Health and Sport Studies from Miami University in 2004, he went on to play professional baseball in the New York Yankees and San Diego Padres organizations before having career-ending shoulder surgery in 2009.  Through his own experience with rehab from surgery, he saw the positive impact physical therapy can have on someone’s day to day function and quality of life. 
In 2015, he earned his Doctorate in Physical Therapy from The University of St. Augustine and has since continued his training to earn his Manual Therapy Certification.  After working in an orthopedic outpatient clinic in Tampa for 2 years, he pursued travel PT opportunities out west, working in Colorado, Arizona, and California before returning to Tampa in 2019.  In his free time, he enjoys traveling and spending time outdoors hiking, biking, and listening to live music.

Jessie Grace

Jessie Grace is a board-certified Family Nurse Practitioner with her Doctorate in Nursing Practice (DNP). She graduated from Union University in 2017 and completed her doctoral research in chronic pain management without opioid use.  She completed over 500 contact hours in her specialty of trigger points and dry needling.  She is passionate about her work with mesenchymal stem cells and regenerative medicine.  She also enjoys performing joint injections, trigger point injections, and dry needling.  She is dedicated to helping patients of all ages and backgrounds improve their quality of life and live their best life. 


Meagan Johnson, DNP, APRN, FNP-BC is a Florida native and a proud “triple Gator”. Meagan holds two Bachelor’s degrees and a Doctorate from the University of Florida in Health Science and Nursing.

As a Registered Nurse, Meagan worked in the Trauma Unit at UF Health Shands for over five years. Her time as a bedside nurse inspired her to return to school to become a nurse practitioner. 

Meagan ultimately earned a doctoral degree in nursing from the prestigious University of Florida. She graduated with a Doctor of Nursing Practice with a focus on family medicine. 

As a board-certified Family Nurse Practitioner, Meagan is passionate about patient-centered care with a holistic approach for patients of all ages. Her interests are in orthopedics and physical medicine. 

In her spare time, Meagan enjoys spending time with her family and fiance, traveling, cooking, and cheering for the Florida Gators. She is excited to be a part of the team at Florida Spine and Injury!

Erin Leegan

Dr. Erin MCGraw Leegan joined the Ethos Health Group Team in February 2020. Dr. Leegan has been a licensed physical therapist for over 6 years, in addition to her educational years. She specialized in manual therapy, after being specifically mentored under a manual therapy enthusiast for two years post graduate school, this training encompasses a focus on Maitland and Chad Cook techniques. Currently, she is passionate about her rehab for patients that have hand/feet neuropathy, traumatic brain injuries, knee injuries, and multiple other musculoskeletal impairments.

Her interest in physical therapy peaked while she was a competitive swimmer in her youth. She viewed the efficacy of physical therapy firsthand through multiple shoulder and knee injuries among high school and collegiate teammates. She realized that physical therapy is a career to help others restore their daily function, and therefore she wanted to be that person that helps others continue to do what they love to do. 

Dr. Leegan was a nationally ranked swimmer throughout high school and college. She swam for Colgate University from 2007-2011 and was captain of her Division I swimming program. Among multiple school and Patriot league records, she received the prestigious award of Colgate Female Athlete of the Year and Patriot League Swimmer of the Year in 2011. After finishing her career as a collegiate athlete, she continued to pursue her passion for fitness, wellness, and health through the profession of physical therapy. 

Dr. Leegan currently resides in Daytona Beach, FL. She loves the beach, paddle boarding, swimming, and running on a regular basis. She lives with her husband & two young kids (2 boys-both under the age of 4). She is excited to be a part of such a caring, supportive, and patient-focused company.


Brittney Lewis-High-Quality

Dr. Brittany Lewis lives in St. Pete and is a Traumatic Brain Injury Tester for the Tampa Bay area offices. She enjoys testing and working with patients to help paint a picture toward their rehabilitation goals. 

Dr. Lewis went to school in Connecticut at the University of Bridgeport School of Chiropractic. She graduated magna cum laude and received the Clinical Excellence Award, honored in recognition of outstanding patient care. Dr. Lewis also teaches yoga and enjoys the close relationship between her two careers. Feel free to show off your down dog when you meet her.



Diana Linn, PT, DPT, earned her Bachelor of Science Degree in Biological Sciences in 2008 from the University of Maryland, Baltimore County. She continued her education at the University of Maryland, Baltimore, earning her Doctorate in Physical Therapy in 2011.

Diana has spent most of her Physical Therapy career treating and working with seniors. Diana is
passionate about using her background of movement science and wellness to create individualized plans for her patients so they can return to the activities they love.

In her free time, Diana enjoys yoga, being outside, and spending time with friends and family.



Lananh Nguyen attended Nova Southeastern University where she graduated with her Doctor of Physical Therapy in May of 2020. She enjoys working with patients and hearing their interesting stories, while helping them reach their goals. Some of her hobbies include photography and eating good food.


Dr. Justine Nierras earned her Bachelor's Degree from Silliman University in the Philippines and her Doctorate in Physical Therapy from Utica College in New York. She has been practicing for four years and prides herself on helping patients improve their outcomes through neurologic and balance training.

She loves when patients are able to independently do balance tasks that they needed assistance with previously, and when they realize and report to us that they can do daily activities with less effort. It is very rewarding to see them enjoy life more because they feel better, and that they know that they got there because of the effort and dedication they put in. That empowerment is what we try to give each and every patient.

Outside of work, Dr. Nierras enjoys reading, hiking, and rock climbing.



Dr. Jessica Perhealth was born in Ocala, FL and graduated from Belleview High School. She was an avid member of the softball team which led to her excelling her athletic career at Central Florida State College, followed by Bluefield State College, where she graduated with a Bachelor of Science degree in Interdisciplinary Sciences and a minor in Biology.

While at Bluefield State, Dr. Perhealth was a dedicated softball and volleyball player, president of the Pre-med Club and the Newman Club, a member of the Delta Chi Omega sorority, spent time volunteering at a local chiropractor's office as well as at the area hospital.

Dr. Perhealth decided to follow her love of healthcare and earned her Doctor of Chiropractic degree; graduating from Palmer College of Chiropractic in Port Orange, FL in March 2012.

Dr. Perhealth uses Palmer package adjusting technique: Gonstead, Diversified, Thompson, as well as Flexion/Distraction and activator.

Jessica and her husband, Mike Perhealth, reside in Summerfield with their four children, Myer, Paisley, Wyatt and Westin. They are both local residents of Marion County and are proud to serve their community.

Chandra Ramos-High-Quality

Chandra Ramos, APRN, DNP is a "double Gator" receiving her bachelor's degree and doctorate in nursing practice (DNP) degree from the University of Florida. 

She is a certified family nurse practitioner as well as a member of the American Academy of Nurse Practitioners and the Florida Association of Nurse Practitioners. 

She has called North Central Florida home since 2006. In her spare time, she enjoys exploring Florida State parks with her husband and two daughters. 

Caitlyn Royal

Caitlyn Royal, DPT, is a compassionate and motivated Doctor of Physical Therapy providing
rehabilitative services to Sarasota and Hillsborough counties since August 2019.

She earned her Bachelor of Science in Health Sciences with a minor in Therapeutic Recreation from Utica College in Central New York before moving to South Florida to pursue her Doctorate Degree in Physical Therapy.

She graduated from Florida Gulf Coast University in May 2019, eager to follow her dream of helping
others as a Physical Therapist.

Caitlyn joined Ethos Health Group to help those living in our community who seek to enrich their lives through improved mobility and confidence while building lasting therapeutic relationships.

Kent Sabatose-1

Dr. Sabatose, D.C., D.I.C.B.N., grew up in a small, rural Pennsylvania town called Brockway.

In high school, he played soccer, basketball, and cross country. He completed his undergraduate studies at Allegheny College where he majored in Biochemistry and minored in Psychology.

Dr. Sabatose attended chiropractic school in Port Orange, FL at Palmer College of Chiropractic where he graduated as the Valedictorian of his class. Dr. Sabatose has undergone post-graduate specialization in the field of Clinical Neuroscience, and he has his board certification from the International Board of Chiropractic Neurology.

Dr. Sabatose enjoys going to the beach, eating, and spending time with family and friends.

Julia Sancarranco-High-Quality

Julia “Andie” Sancarranco, MSN, APRN, AGPCNP-BC, completed her Bachelor of Science in Nursing at Methodist College of Nursing, Peoria, Illinois. She enjoyed working in various specialties including Neurosurgery at Houston Methodist Hospital in Houston, Texas and Neurology at Anne Arundel Medical Center in Annapolis, Maryland. 

Andie went on to earn her Master of Science degree at Vanderbilt University, in Nashville, Tennessee. She holds a national board certification with the American Nurses Credentialing Center (ANCC) as an Adult Gerontology Primary Care Nurse Practitioner. 

Andie is passionate about improving the quality of life and reducing pain in her patients. Outside of work, Andie enjoys spending time with her family and two dogs Cooper and Clyde. 


Matt Shanklin, DPT, LMT, earned his Doctorate in Physical Therapy from Nova Southeastern and has
practiced in Tampa Bay his entire career.

Previously working in sports massage for distance runners and triathletes, Matt specializes in manual therapy for all musculoskeletal injuries. He has worked closely with leading Chiropractors in Tampa Bay to best understand the needs of injury patients as well as those with specific nerve pathologies including Neuropathy.

Joining Ethos in St. Petersburg and Palm Harbor, he employs an evidence and experience-based
approach to patients suffering from Neuropathy, Knee Pain, and injuries sustained from accidents and trauma.

Shanklin lives in St. Petersburg with his wife and daughter and can be found participating in local triathlons and spending time with his family.

Craig Siefkes-High-Quality

Originally from central Kansas, Dr. Craig Siefkes attended Kansas State University, graduating with a bachelor’s degree in Psychology. He continued on to Palmer College of Chiropractic in Davenport, IA, where he received his Doctor of Chiropractic in 2009.

Dr. Siefkes has practiced in central Pennsylvania, Austin, TX, and central Florida. He is certified in Activator Methods and enjoys learning more about Thompson technique.

In his free time, Dr. Siefkes enjoys going to Disney World with his wife and daughter, as well as watching hockey, college football, and theater productions. He is also a huge fan of Star Wars, Star Trek, and Doctor Who.



Dr. Joseph Soto attended Life University, graduating with his Doctorate in Chiropractic in 2015. He primarily works with patients who are experiencing pain due to motor vehicle accidents or other personal injuries and is experienced in nerve conductive velocity testing. His favorite part of working with patients is helping to relieve their pain and increase their daily quality of life. Outside of the office, Dr. Soto enjoys spending time with his son.


J Torres 1

Jonathan Torres grew up in Elizabeth, New Jersey where he learned to love competitive sports, eventually playing division II college baseball.

He received his B.S. degree in Biology from Bloomfield College in Bloomfield, NJ and his Doctorate of Chiropractic degree from Palmer College of Chiropractic in Florida. As our Personal Experience Manager, his love for his profession is expressed through the care and compassion he shows our patients. 

Jonathan resides in Ocala, Florida with his wife and three young children, including their daughter and twin sons. He is very grateful to have such a satisfying life and a supportive community. He spends his free time playing golf and eating as much Cuban food as he can get his hands on.

Jen W

Jennifer has earned her Masters in Science of Nursing through South University. She is certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners (AANP). Though she is a recent FNP graduate, January of 2020, she has a strong background working over 11 years in Emergency/Critical care, medical and surgical nursing, and even rehabilitation. She enjoys living with her family in Edgewater, FL; where she has resided the majority of her life. She enjoys being outdoors in the Florida sun and entertaining her 3 children. She believes in developing a strong rapport and trusting relationship with her patients in order to provide them with the care they deserve. She ensures every individual gets specialized care tailored to meet their individual needs. 

Heather Watford-High-Quality

Heather Watford joins our practice with provider experience in family medicine, urgent care, and wound care. Originally licensed as a RN for 22 years, Heather worked in various intensive care units, recovery room, and emergency room before earning her MSN as a Family Nurse Practitioner.
As a graduate from Florida Southern College, Heather brings a high level of academic achievement and a caring approach to patient management. Her goal is to promote a healthy lifestyle and ensure each patient feels valued. She also enjoys teaching other students as a clinical preceptor. 


Heather enjoys spending time with her family and enjoying the outdoor activities available in Florida. Her passion is serving on medical missions teams providing medical care to underserved populations across the globe. 


Winona Werts

Winona Werts, APRN-C, FNP, grew up in Rockledge, FL. She attended the University of South Florida where she received her Bachelor of Science in Nursing Degree.

She served as an RN in Columbus, OH for 5 years, caring for pediatric patients at the Nationwide Children’s Hospital. She continued her
education at the University of Cincinnati where she completed her Master of Science in Nursing
Degree in the Family Nurse Practitioner Program.

As a Nurse Practitioner for Ethos Health Group, Winona brings with her a background in Internal
Medicine and Primary Care.



Rich is a Michigan transplant and has resided in Florida for the past several years.

He is a licensed Physical Therapist certified in Mckenzie mechanical diagnosis and therapy practicing since 2002. 

He attended school in The Netherlands.

An interesting fact about Rich is that he is a book lover, he has traveled the world abroad and he has a black belt in two martial arts.  

He has also been able to practice Yoga in several different countries!


Graduating in one of the inaugural classes at the Florida campus of Palmer Chiropractic College Dr. Joseph Wolfersberger, D.C. has been in practice for over a dozen years. Chiropractic has been part of his life since childhood in a small beach town on the Jersey Shore. In his senior year in high school a rough tackling drill at football practice caused a severe “whiplash injury” that required a longer course of treatment. It was a wonderful experience recovering from the injury, see others get better. This great experience set him on the path to becoming a Doctor of Chiropractic.

After high school, he decided to relocate to Jacksonville to attend the University of North Florida. After earning a bachelor's degree in Spanish from the University of North Florida, he worked in the banking and communication industries until a layoff from a job led him to reassess his career choice. He went back on his original path to becoming a chiropractor.

After becoming a Chiropractor, he was hired on with one of the most respected, experienced, and longest continuously operating clinics in the state of Florida. He then covered at several different chiropractic offices around North and Central Florida until finding his perfect fit with Florida Spine and Injury in Lake City.

Dr. Wolfersberger has 3 daughters, two of whom are adults and one pre-teen still at home. In his spare time he likes to do yoga, outdoor activities such as camping, hiking, riding bikes and kayaking/canoeing. Being a “beach kid” with shoes full of sand, he loves spending time at the beach or at one the many springs around northern and central Florida whenever possible.



Dr. Wood, D.C., grew up in Lake City and has practiced in Lake City and Live Oak since 2003. He
graduated with a Doctor of Chiropractic Degree from Life University in 2001.

He is a member of the Florida Chiropractic Association and North Florida Chiropractic Society.  The main focus of his treatments is auto accident injuries, disc injuries, and neck and back pain.

He enjoys keeping up with the latest research and advancements in the treatment of whiplash injuries.

Dr. Wood is married with two children and enjoys playing golf and fishing in his free time.
